Annual Notice of Open Enrollment meetings for the July 1, 2014-June 30, 2015 plan year

Location: Cox Family Room, Ford hall
Dates/times: Monday, May 5th from 10:00 am – 3:00 pm
Tuesday, May 6th from 11:00 – 2:00 pm

Come as your schedule permits.

The meetings will cover: Health insurance open enrollment –Health New England – enrollment and/or changes; dental open enrollment in Delta Dental; Flexible Spending Accounts (FSA) enrollment; Life, AD&D and Long Term and Short Term Disability with Mutual of Omaha.

All benefit eligible employees are required to fill out a benefit election form even if you are not participating in our plans; if you waive health insurance you must also fill out a health insurance responsibility disclosure form (HIRD) per the state of Massachusetts’ health insurance requirement.
